We are required to have your insurance information 48 hours prior to your appointment. This gives us time to verify your coverage and benefits. Please note that you will be responsible for paying services at the private pay rate should you fail to notify us of your insurance information 48 hours before your appointment.
If you choose to meet with your provider via telehealth, please note that your insurance plan may not cover virtual appointments. It is best to follow up with your insurance company regarding if this option is available for you. If your insurance fails to cover telehealth services, you are responsible for paying for services at the private pay rate.
Please note that the copayment insurance rates suggested by our office can change daily and is not a guarantee of payment by your insurance company. If for any reason, the insurance company fails to submit payment for services rendered, you will be responsible for covering services at 100%. If you have any questions about copayment information, it is advised to contact your insurance company directly. We will try to keep you updated as much as possible. If your insurance changes for any reason, you are responsible for notifying the office promptly so that we can submit claims to the appropriate insurance provider.
We genuinely understand that life can be overwhelming and unexpected. As a result, we offer both 48 and 24 hour appointment reminders, in addition to an appointment confirmation.
Please note that appointments must be cancelled or rescheduled before 24 hours of your scheduled appointment. In addition, if you are more than 15 minutes late, you will be considered a 'No Show' and will be charged the full out-of-pocket rate of your scheduled service. If your clinician is more than 15 minutes late, they will call and may re-schedule at no additional fee to you.
Any appointments cancelled or rescheduled within the 24 hour will be charged 100% of the out-of-pocket rate for your scheduled service. Please note that 'No Shows' are charged 100% of the out-of-pocket rate for your scheduled service. Refunds are not given.
